Prepare and Save Time
Your initial visit will involve a comprehensive oral health evaluation that should take about an hour. Download and complete these registration forms, and bring them to your appointment. We’ll expedite registration and answer all of your questions.
Insurance
We file most insurance claims electronically and will work with you to make the most of your benefits. Please prepare to pay the estimated portion of your cost at your appointment. We accept assignment of benefits, so your insurance provider will pay us directly. If a balance remains after the insurance company remits payment, we will adjust your account accordingly.
Payment
Pay by cash, check, American Express, MasterCard, or Visa. Financing is also available through Chase Health Advance. Click on the logo below to apply for financing online.
Postoperative Instructions
